Below is a copy of our New Patient Information Form. If you do not have an appointment set to see Dr. Sarah, please call our office and we'll be happy to schedule your first visit. We are open Monday, Tuesday, Wednesday, Friday, Saturday so stop in or give us a call at 517.347.2222.
If you have already made your appointment, please feel free to download a copy of our New Patient Information Form. Having it completed before your first visit can help with getting everything set up so you will be all ready to start on your way to optimum health.
